Consumer Information & Client Rights (Texas Law)

In accordance with Texas House Bill 4224 and Section 181.105 of the Texas Health and Safety Code, the following information is provided to ensure transparency and to inform consumers of their rights.

Requesting Your Health Care Records

Clients have the right to request copies of their health care records.

To request your records, please submit a written request by email or in writing to this practice. Requests should include your full name, date of birth, the specific records requested, and your preferred method of delivery. Requests will be processed in accordance with Texas law and applicable privacy regulations.

Contacting the Texas Behavioral Health Council

Clients may contact the Texas Behavioral Health Council for questions regarding licensure, professional standards, or regulatory matters.

Filing a Consumer Complaint

Clients who wish to file a complaint regarding services provided may do so with the Office of the Attorney General of Texas – Consumer Protection Division.
